The full picture

The Eversense 365 is unlike any other glucose sensor: instead of a patch you stick on and replace every week or two, it is a small fluorescence-based sensor — under three-quarters of an inch long — that a trained clinician slides just under the skin of your upper arm in a few-minute in-office procedure, where it stays and reads glucose for a full year.12 It is the world's first one-year CGM.1 A separate removable, rechargeable transmitter sits on the skin over the sensor (held by a gentle silicone adhesive you change daily), powers the sensor, and sends a reading to your phone every five minutes.3 Because the transmitter comes off, you can take it off to shower or charge it without wasting the sensor.3

How accurate is it? In the ENHANCE pivotal study of 110 adults, the Eversense 365 had an overall MARD of 8.8% across the entire year — meaning readings landed, on average, within about 8.8% of lab glucose — while needing only about one calibration per week.4 According to PubMed, ENHANCE also reported a 96.6% confirmed alert-detection rate at 70 mg/dL (low) and 97.9% at 180 mg/dL (high), and 90% of sensors survived the full 365 days (DOI).4 In the low range specifically, the manufacturer reports a mean absolute difference of 7.7 mg/dL and detection of 91% of lows at 60 mg/dL.5 A real strength of deep-tissue placement: virtually no "compression lows," so far fewer false alarms when you lie on it overnight.5

Wear, warm-up and calibration. One insertion lasts up to 12 months.2 After the sensor is linked to the transmitter there is a 24-hour warm-up with no readings, then four fingerstick calibrations spaced 2–12 hours apart before glucose displays.67 You then calibrate daily for about the first 13 days, after which it drops to roughly once a week for the rest of the year — the lightest calibration burden of any Eversense generation, though still not fully calibration-free like Dexcom or Libre.15

Alerts. Uniquely, the transmitter gives on-body vibration alerts for highs, lows and out-of-range — you feel them on your arm even if your phone is in another room — and the app has both high and low predictive alerts to warn you before you cross a threshold.57

What it works with. It is iCGM-classified, and in 2026 it became the first implantable CGM used in automated insulin delivery, paired with the Sequel twiist pump.89 Data goes to iPhone and Android, with a snapshot on Apple Watch as a secondary display.9 There is no ketone sensing and no open-source/DIY loop support.5

Who it's for, and where. Cleared for adults 18+ with type 1 or type 2 diabetes; not for use in pregnancy.27 It is sold in the US (launched late 2024) and is now CE-marked in the EU (January 2026), with rollout to Germany, Italy, Spain and Sweden underway.10 Most US commercial plans and Medicare cover it for people on insulin (Medicaid does not), and the PASS assistance program can keep commercial out-of-pocket low for a full year, not counting the insertion/removal procedures.11

What's coming. The line has marched from 90 to 180 to 365 days,12 and the near-term direction is wider AID integration — twiist is the first pump partner, with the company signalling more to come — plus the European launch now opening up.810
