Medtronic's first disposable, all-in-one CGM — half the size of its older Guardian sensors, factory-calibrated, 7-day wear. The standalone version pairs with the InPen smart pen rather than a pump. Independent head-to-head data put its real-world accuracy behind Dexcom G7 and Libre 3, though it detects lows unusually well.
The scorecard
Manufacturer MARD ~10.2% vs venous reference, but the only independent head-to-head found 11.6% (venous YSI) rising to 16.6% (capillary) — and two further real-world studies reported MARDs of 13.8% (exercise) and 19.9% (flight), consistently behind G7 and Libre 3.[1]
Interstitial lag like all sensors; independent data show it lags and reads low during fast-rising glucose (MARD ~20% when rising >2 mg/dL/min), a meaningful weakness post-meal.[1]
Standalone Simplera pairs only with the InPen smart pen / Smart MDI app; no commercial AID and no open-source loop support (the pump-driving sensor is the separate Simplera Sync). Narrowest ecosystem of the modern sensors.[5]
7-day wear — shorter than Libre's 14 days or the 15-day G7; independent sensor-survival to day 7 was only ~81.5%.[1]
Factory-calibrated; no fingersticks required. Optional manual calibration is available but not needed for normal use.[1]
Configurable high/low alerts plus predictive "alert before low" up to 60 min ahead, on phone and Apple Watch; in the independent head-to-head its low detection was class-leading (93.1% of true lows, vs 73–80% for G7 and Libre 3) while its high detection (84.6%) trailed both rivals (~99.7%).[1]
Disposable all-in-one sensor about half the size of the Guardian 4, worn on the upper arm, no overtape; 2-hour warm-up.[5]
No ketone sensing — glucose only.
CE-marked and sold across Europe since 2023–24 and FDA-cleared (Aug 2024) with a phased US rollout; US copay assistance can keep out-of-pocket cost low, but availability still varies by region and the standalone ecosystem is small.[9]
The full picture
The Medtronic Simplera (standalone) is Medtronic's first disposable, all-in-one continuous glucose monitor (CGM) — a small square sensor you press onto the back of your upper arm with a one-step applicator, with no separate transmitter and no overtape. It's about half the size of Medtronic's older Guardian 4 sensor.1 It reads glucose every five minutes and streams it by Bluetooth to a phone app and Apple Watch.2 Two versions exist and it's important not to confuse them: the standalone Simplera here pairs with the InPen smart insulin pen (the "Smart MDI" system) for people on injections, while a separate sensor, the Simplera Sync, is the one that drives the MiniMed 780G pump.1
Accuracy — read this carefully. Medtronic's own data put the MARD (mean absolute relative difference — lower is better) at about 10.2% against a venous lab reference.3 But the only manufacturer-independent head-to-head study — 24 adults with type 1 wearing Simplera, Dexcom G7 and FreeStyle Libre 3 at once — found Simplera less accurate than both rivals: MARD 11.6% against the venous YSI analyser, rising to 13.9% against another venous method and 16.6% against a fingerstick meter, versus roughly 9–12% for G7 and Libre 3.3 Two further real-world studies reinforced the gap: a MedARD of 13.8% around exercise and 19.9% during air travel, both significantly worse than G7 and Libre 3.45 Accuracy is worst in the first 12 hours (MARD ~20%) and when glucose is rising fast, where it tends to read low.3 The redeeming strength: Simplera detected lows better than either competitor (93% of true lows caught), so it leans toward catching hypoglycemia at the cost of more false alarms.3
Wear, warm-up & calibration. Each sensor lasts 7 days — shorter than Libre's 14 or the 15-day G7 — with a 2-hour warm-up, and it is factory-calibrated so no fingersticks are required (an optional manual calibration exists but isn't needed).32 Independent testing saw only ~82% of sensors survive the full 7 days.3
Alerts & app. The app gives configurable high/low alerts plus a predictive "alert before low" up to 60 minutes ahead, a fixed urgent-low alert at 64 mg/dL, and a live Time-in-Range view; alerts mirror to an Apple Watch.62
What it works with. The standalone sensor connects only to the Simplera/InPen Smart MDI app on iOS 15+ or Android, plus Apple Watch.2 It does not drive any commercial automated insulin delivery (AID) system and is not supported by any open-source/DIY loop — that's the separate Simplera Sync with the 780G.1 This narrow ecosystem is its biggest practical limitation.
Ketones. None — it senses glucose only, like all current mainstream CGMs.
Who it's for. The standalone Simplera CGM is indicated for adults 18 and older.6 (The Sync sensor is cleared from age 7.)7
Access & cost. Simplera has been CE-marked and sold across Europe since 2023–24 and won FDA approval on 7 August 2024, with the enabling InPen app cleared in November 2024 and a phased US rollout starting with existing customers.718 In the US, Medtronic advertises copay assistance with automatic savings to keep out-of-pocket cost low.9 Availability genuinely varies by region, so confirm locally.
What's coming. The near-term story is the broader US launch of the standalone Simplera plus Smart MDI now that the app is cleared,8 and Medtronic has signalled longer wear and accuracy gains for the platform — both of which it needs to close the gap to Dexcom and Abbott.
