A removable, two-part tubeless insulin patch pump for adults on multiple daily injections, FDA-cleared in April 2026 and commercially available in the US since late June 2026 in a deliberately phased rollout. The full picture

Pivot is not trying to be the fanciest pump. Its stated target is the "almost-pumper": adults who rely on injections because traditional pumps feel complex, expensive or cumbersome. The interesting design choices are simplicity, removability, 300-unit capacity and controller-light bolusing. The limitation is just as important: it is an insulin pump, not yet a closed-loop ecosystem.

It is now shipping. On 24 June 2026 Modular Medical announced that Pivot is commercially available in the US, with first shipments beginning that week — describing it as "only the second fully electronic, tubeless insulin pump available in the United States".1 That follows the FDA 510(k) clearance granted on 9 April 2026.23 So this is no longer a pipeline device: real people are wearing it.

But "available" here means a staged rollout, not a national launch. Modular Medical is deliberately starting narrow — a first wave through selected high-volume endocrinology practices, expanding across multiple metropolitan markets by late 2026, with fuller commercial expansion activity from the fourth quarter of 2026.1 If you are not near one of those early practices, the practical answer to "can I get one?" in mid-2026 is still probably "not yet". Payer coverage is likewise still being worked out, and there is no approval outside the US.

Who it is cleared for. The FDA clearance covers adults with type 1 or type 2 diabetes who are currently on multiple daily injections — it is not a T1D-specific device, and it is not cleared for children.3

What it still is not: an automated system. Nothing in the clearance or the launch announcement claims CGM pairing or an automated-insulin-delivery algorithm.31 Every dose is still your decision. For someone coming off injections, that may be exactly the point — Pivot is pitched as a gentler first step onto a pump, not as a competitor to a closed loop. But if what you want is a system that adjusts insulin for you overnight, this is not that, and choosing it means choosing manual dosing in a tubeless package.
